Open the service request form URL provided by your organization. If the page asks for a password, use the password provided by the form administrator.

  1. Under Select the category for issue, select the category that suits your issue. If there is only one option, it is selected automatically.
  2. Fill in Your name, Your email and the necessary contact details.
  3. Enter The title of the issue you are reporting and A more detailed description of the issue.
  4. Select the device, or Spot, under Choose target if the form includes a Spot selection field. You can search for a target by typing part of its name. If a tag surface number field is available, you can identify the target by entering the identifier shown on the device according to the field's instructions. When filling in the form manually, use only one of these methods.
  5. Fill in the category's additional fields and select any required checkboxes. Add the necessary attachments using the Add files button or by dropping them into the attachment area.
  6. Check the information, select Create a new service request and wait for the success message.

In the description, explain what is happening with the device, where it is located and when you noticed the issue. Check your email address particularly carefully, as it will be used for further communication about the request.

Not all forms display the same fields. The category may change the additional fields and whether a Spot is required. A form opened via a tag may already contain the device's identifying information. Prefilled tag and target details that refer to the same target may remain in both fields. If you switch categories and then switch back on the same page, the additional information you previously entered for that category is restored.

You can change the form's language using the flag menu at the top. The basic and additional information you have entered is retained when you change languages in the same tab, provided your browser allows temporary storage of this information. Check the target selection and add the attachments again after changing languages.

Attachment count and size

You can attach up to 25 files. The size limit for a single file is approximately 1 GB. Common document, image, audio, video, archive and text files are allowed, such as PDF, JPG, PNG and ZIP files.

Files that are too large or attachments exceeding the 25-file limit are not added to the attachment list. Before submitting, check that all the necessary files appear in the list. If an attachment error is reported during submission, remove or replace that file and submit the form again. Attachment errors prevent the request from being saved.

Correcting errors and confirming submission

If the form indicates that information is missing, fill it in before submitting again. If you see the message Use either the tag surface number or the target selection, not both., clear one of the target fields. You can clear the target selection by selecting the empty option in the list. If the target selection is required, keep it and clear the tag surface number.

The message Service request was successfully sent. on the success page confirms that the request has been submitted; it does not mean that the reported work has already been completed. Keep the email you receive for any further communication. You can start another request using the Add another link.
